- Your personal data: first and family name, PESEL number, sex, sensitive data contained in your medical records will be processed pursuant to Article 6.1.c of the GDPR, for the purposes of providing medical services. Your contact data (phone number, e-mail address) will be used by the health facility to contact you, including to confirm or cancel an appointment or to send medical records.
- Please be informed that histopathology tests are performed by an outsourced company. Data will be revealed pursuant to a data processing agreement.
- In association with the processing of your personal and contact data , you have the right to request the CONTROLLER to: access, rectify, delete or limit the processing of your personal data, except the data that are contained in medical records. It is mandatory to store medical records for 20 years.
- Revealing your personal data is voluntary, but if you refuse to reveal your data, you will not be able to receive any medical services. Please, keep us informed of any changes in your personal or contact data.
